Submit a Support Request Form

1. Contact Support with a completed HIPAA Authorization form (linked below).
2. Once processed, your medical record will be sent to you via email.

Message Center Request

1. Login to the Dashboard
2. Send a message through the Message Center requesting your medical record.

3. Be ready to provide the email address associated with your account.
4. Complete and return the HIPAA Authorization Form sent to you via email.
5. Once processed, your medical record will be sent to you via email.
